CONSENT TO TATTOO FORM

please respond to the following statements 

i am over the age of 18, of sound body and mental state, and not under the influence of any substances, legal or illegal. I am not pregnant, or nursing. I do not have any medical or skin conditions that could affect the tattoo or the artist.
I understand the getting a tattoo will permenantly mark and alter my skin. I understand that because of the nature of the anataomy, the design may have variations in colour and shape. I understand that the tattoo itself will change over time.
All precautions have been taken by the pop ink and it's artists, but infection is always a possibility. Following the artists aftercare will greatly reduce the chance of infection, and it is your responsibility to take care of your new tattoo. I agree to pay for any touch ups needed due to my negligence.
i give permission to POP ink and the artist to use photos and videos of the procedure and or the tattoo. I consent to the tattoo and the procedure and agree to accept all risks and responsibilities. i have read and understood this form and waive and release POP ink and the it's artists form all liability
